You can begin with a regular MS Word Document.
Add fields where you would like to collect data.

The tool bar for forms is under "View"

The "ab" is the field box for text. Add ASCII tags as below.
You can color the « » white to hide them, but leave the field
shaded so the end user can see it.

Double click on the field to do data pre-qualifying,
(i.e. set it as text, number or date, put in sample data, etc.).

You can lock the document on the "Forms toolbar,"
but it is better to set a password in the "Protect Document"
part of your tool bar.

To put a live link in the document (for the dropbox) or put an "open/unlocked"
section to your document (for inserting a digital certificate, image, spreadsheet,
object...) you must insert a "Break"

Choose "Continuous"

Click OK then go to "Sections"
on the "Protect Document" menu.

Un-check the section you want unlocked.

Click OK. You now have a locked document,
add a password if you like.

If you use the "lock" on the forms toolbar,
anyone can unlock it.

To send a form to multiple tables add the following tags to the document where you
would like the data to go to another table

ªºtablenameºª     <-- the ª is "Alt 166" the º is "Alt 167"

This will supercede the one form to one table routing of AutoData.
You can color this tag "white" to hide it.
